Just read this article on the Swiss model.
It's a private insurance with managed costs that works partly by mandating that:
a) everyone have insurance and
b) there be a base plan that doesn't make insurers any money.
You can build the extras on that model to get as fancy as you want (spend as much extra as you want).
Does that almost sound acceptable to Americans?
