The U.S. Defense Department cannot account for about $2 billion it was given to cover Iraq-related expenses and is not providing Iraq with a complete list of U.S.-funded reconstruction projects, according to two new government audits.

Well duh, doesn't seem adequate given that this true before the Iraq war took place even according to audits of the dod.
 
This isn't at all new. Rumsfeld announed on 9/10 that the Pentagon has lost over $2Trillion. AFAIK neither Bush nor Obama has put policies in place to account for or prevent this type of thing from happening again.
